How to Use the Fertility Awareness Method for Birth Control (Naturally and Confidently)

A Natural Shift in Birth Control
More and more women today are seeking alternatives to hormonal birth control — whether due to side effects, personal values, or a desire to reconnect with their body’s natural rhythms. But what if there was a method that was backed by science, natural, and put you back in charge?
Enter: the Fertility Awareness Method (FAM) — a hormone-free, evidence-based approach to birth control that teaches you how to understand your body’s signs of fertility, so you can confidently avoid or achieve pregnancy on your terms.
In this week’s episode of the Prepare for Pregnancy Podcast, I sat down with Hana, fertility awareness educator and founder of Chart Your Cycle, to dive deep into the Fertility Awareness Method (FAM). Together, we unpacked how this natural birth control method works, why it’s often misunderstood, and how you can confidently use it to avoid pregnancy — without synthetic hormones.
What Is the Fertility Awareness Method?
The Fertility Awareness Method (FAM) is a natural birth control method based on tracking your body’s fertility signals — also called biomarkers — like:
- Cervical fluid (aka cervical mucus)
- Basal body temperature (BBT)
- Optional: cervical position or LH test strips
By observing and charting these signs, you can confidently identify which days of your cycle you are fertile and which days you are not. On fertile days, you can either abstain from sex or use a barrier method like condoms. On non-fertile days, you can have unprotected sex without fear of pregnancy.
FAM isn’t just “counting days” — it’s based on real-time, biological data from your body, not predictions or algorithms.
FAM vs. the Rhythm Method: What’s the Difference?
Let’s bust one of the biggest myths right away:
The Fertility Awareness Method is NOT the same as the rhythm method.
The rhythm method relies on past cycle averages to guess your fertile window. FAM uses real-time data (like mucus and temperature shifts) to identify ovulation as it happens. That’s why FAM is much more effective — and far more empowering.
How Effective Is the Fertility Awareness Method?
You might be surprised to learn that FAM, when practiced correctly, is extremely effective:
- Perfect use: up to 99.6% effective
Compare that to:
- The pill (perfect use): ~99.7% effective
In fact, here is a chart laying out the standard Birth Control Methods in relation to FAM.
| Birth Control Method | Perfect Use Effectiveness |
|---|---|
| The Pill | 99.7% |
| Fertility Awareness Method | 99.6% |
| Condoms | 98% |
| IUD (Hormonal) | 99.8–99.9% |
When women learn FAM from a trained instructor and commit to consistent daily tracking, it’s a highly reliable method of preventing pregnancy.
Why Choose FAM for Birth Control?
Women choose fertility awareness for a variety of reasons:
- Hormone-free: No synthetic estrogen or progesterone
- Body literacy: Understand your body on a deeper level
- Side-effect free: No mood swings, low libido, or weight gain
- Natural lifestyle: Aligns with holistic and root-cause wellness values
- Health insight: Your cycle reveals a lot about your health
And perhaps most importantly: it puts you in the driver’s seat.
How to Use the Fertility Awareness Method for Birth Control: Step-by-Step
Let’s break it down into actionable steps.
Step 1: Learn the Method from a Certified Instructor
Reading a book like Taking Charge of Your Fertility is a great start, but to use FAM confidently as birth control, it’s best to learn from someone who can help you interpret your own charts, troubleshoot irregularities, and personalize the method to your cycle.
I offer 1:1 FAM Coaching — you can learn more right here.
Step 2: Track Cervical Fluid Daily
Cervical fluid (aka cervical mucus) changes throughout your cycle. You’ll want to observe it every time you go to the bathroom. Here’s a quick guide:
| Type | What it Means |
|---|---|
| Dry or sticky | Infertile |
| Creamy or lotiony | Possibly fertile |
| Egg-white or watery | Highly fertile |
Once you notice your cervical fluid becoming more slippery/stretchy, you are approaching your fertile window.
Step 3: Take Your Basal Body Temperature (BBT)
Each morning, take your BBT with a special thermometer before getting out of bed. After ovulation, your temperature will rise slightly and stay elevated until your next period.
This confirms ovulation and helps you identify the end of your fertile window.
Step 4: Chart Your Data
Use a non-predictive app (like Read Your Body) or a paper chart to log:
- Cervical fluid observations
- Daily BBT
- Optional: LH test strips or cervical position
Over time, you’ll see a pattern and can confidently determine which days are safe for unprotected sex and which days are not.
Step 5: Avoid or Use Protection During the Fertile Window
Your fertile window typically includes the 5 days before ovulation + ovulation day. During this time, if you don’t want to conceive:
- Abstain from sex
- OR use a barrier method like condoms
Outside of this window, unprotected sex is safe and non-fertile.
Step 6: Practice for 3+ Months Before Relying on It Alone
While you can technically start using FAM right away, it’s wise to practice charting for 3 cycles while using backup protection.
This allows you to:
- Build confidence
- Learn your patterns
- Work out any kinks with guidance
Can You Use FAM If You Have Irregular Cycles or PCOS?
Yes — and it’s actually more useful for women with irregular cycles, because it tells you what’s happening in real-time, not based on averages.
Even if you have PCOS or cycle disruptions, FAM can help you:
- Detect ovulation
- Identify hormone imbalances
- Time conception or avoid pregnancy with more precision
A practitioner can help tailor the method to your unique cycle.
Common Myths About the Fertility Awareness Method
Let’s clear up a few more misconceptions:
Myth: You can get pregnant any day of the month
Truth: You’re only fertile for 5–6 days per cycle
Myth: You have to use condoms every time
Truth: You only need them during your fertile window
Myth: It’s too complicated
Truth: Once you learn it, it takes under 2 minutes per day
Bonus: FAM Is More Than Just Birth Control
Even if you’re not TTC or trying to avoid pregnancy, FAM is a powerful tool for:
- Understanding your hormone health
- Spotting red flags early (like short luteal phases or anovulation)
- Preparing your body for conception in the future
- Tuning into your body’s rhythms
Your cycle is your fifth vital sign. Learning to read it is life-changing.
